  • Your contact information (name, organization, telephone number, and e-mail address)
  • Your product and version
  • Your license authorization code
  • Your operating system (e.g. Windows 98, Windows NT 4.0)
  • Your version of Microsoft Office or Excel, if applicable
  • A description of the problem (including steps to re-create, if necessary)
